At Olympic Sports & Spine Rehabilitation we are committed to you and value your time. We strive to make scheduling your appointment as convenient as possible. To schedule an appointment, please contact one of Olympic Sports & Spine Rehabilitation’s six locations.

When scheduling your first visit we will collect basic patient information to begin the registration and insurance verification process. For your convenience, our Patient Registration Packet is available for download or you have the option of receiving the packet by mail, fax or email.

At Olympic Sports & Spine Rehabilitation, you have the option to receive treatment services without a prescription or referral from your physician. However, you need to keep in mind that your insurance carrier may not pay for therapy services without a written referral or prescription. A physician referral is required to receive occupational therapy. To receive a written referral or prescription, please contact your physician.
