Registration

We want to make the paperwork for your initial appointment as convenient as possible by providing you with three options:

• Download, print and complete the Patient Registration Forms:
  • Medical Profile Questionnaire
  • Patient Information
  • Authorization to Release Health Information Orally
  • Clinic Policy & Financial Agreement
  • HIPPA Notice of Privacy Practices
• Please arrive at the clinic 20 minutes prior to your first appointment to complete the Patient Registration Forms if you have not yet already done so.
• We can email, mail or fax the Patient Registration Form to you. and you can fill them out before you arrive for your first appointment. If you wish for us to fax or mail the forms please request this when you call and make your initial physical therapy appointment.

For your initial appointment, you will also need your insurance information and your written referral/prescription for physical therapy.
