Billing Policy

We are network providers with the majority of insurance plans including Regence, Premera BlueCross, First Choice Health Network, Labor and Industries, Medicare, and Tricare. You may be eligible for insurance coverage if you have received a referral/prescription for physical therapy from your physician. As a courtesy to you, we will submit all charges to your primary and secondary insurance companies.

Please keep in mind that many insurance plans require the patient to satisfy a deductible, co-insurance, and/or co-pay as part of coverage. Because of this, we will collect any co-pays by means of cash, check, or credit card at the time of service. We will be happy to verify insurance coverage for your treatment at OSSR prior to your initial examination. We will also let you know if your particular insurance plan requires you to obtain a referral/prescription and/or pre-authorization for treatment. If your injury is due to a motor vehicle accident (MVA), we will bill your Personal Injury Protection (PIP) coverage as a courtesy. However, you will continue to receive a monthly billing statement until your account is paid in full.

If you DO NOT have insurance or a third party is responsible for payment of your injury, please let us know and we will inform you about out “Private Pay Policy.
